Facility Assessment

Service Description

This service will assist you facility in completing facility assessment. The service requires interaction with your interdisciplinary team and leaders. At the end of this service, you will be provided with a completed facility assessment that is required within 1 hour of facility survey beginning November 28 for Phase 2. Consultant will meet with Administrator and provide a list of information that the facility will need to complete, if there are no current process in place.
